Activity Director - back to top
The Activity Director plans, organizes & implements all functions of the Activity Department to meet the standards of regulatory agencies & of this facility & the individual needs of its residents. Must have training & certification or experience as required by the state. Must have ability to read, write & follow oral & written directions at a level necessary to accomplish the job. Must be capable of maintaining regular attendance. Must meet all local health regulations. Must be capable of performing the essential job functions of this job with or without reasonable accommodations.

Medical Records Specialist - back to top
Medical Records Specialist is responsible to maintain active medical records for each resident & see all medical records are completed & preserved to acceptable standards. Assists in establishing P&P for medical records in compliance with facility regulations as well as regulatory statutes.

*Immediate opening: Full-time. Must be Certified Medical Records Specialist and/or LPN with 5-yrs experience in LTC. Must be able to relate positively to residents, families & other staff. RN or LPN Shift Supervisor - back to top
The Nurse Shift Supervisor is responsible to coordinate nursing care to the residents during the designated shift. Also, the Nurse Shift Supervisor is responsible for communicating to the Director of Nursing & facility Administrator incidents/events as related to residents & staff, i.e. injuries, family/resident/staff relations and/or status changes.

*Immediate opening: Full or part time open for discussion. Sat/Sun 7p-7a part-time or Sat/Sun 7p-7a + 1 week day shift to become full-time

Staff Nurse (RN or LPN) - back to top
Nurse must be licensed in the state of TENNESSEE or other compact state and not listed on the abuse registry. Nurse is responsible for directing and supervising personnel on assigned unit and care of residents. Participates in shift to shift reporting of resident status, performs assessment rounds, communicates with physicians and pharmacy regarding resident care needs/changes, family relations, documentation of resident care and other duties as assigned. Certified Nurse Assistant - back to top
The Nurse Assistant, under the general supervision of a licensed nurse, is responsible for providing all activities of daily living according to resident's needs as outlined in plan of care & in accordance facility policy & procedure. Nurse Assistant must be certified in the state of TN or hold certification as a nurse aide in a transferrable state per the State of TN guidelines. The Nurse Assistant must have a genuine interest in the management of direct care for nursing home residents as well as a cheerful attitude, patience & superior use of tact & courtesy in addressing residents, families, staff & visitors.

*Immediate openings: Part-time 7p-7a Sat/Sun shift and PRN.

Environmental Services Aide (Housekeeping) - back to top
Environmental Services Aide is responsible for the overall cleanliness of assigned work areas. Tasks include but are not limited to sweeping, mopping, dusting, of resident units & public areas, stocking, restocking & maintaining inventory for assigned carts & other duties as deemed necessary by the department Director or other managers. Required to follow the schedules, policies & procedures of the department & the facility. Must be able read & write & follow verbal & written instructions. Previous experience working in a health care setting, encountering residents with varying conditions, such as dementia/Alzheimer disease, preferred but not required.

Dietary Cook - back to top
Dietary Cook is responsible to assist in providing quality meals served in a timely manner in accordance with the requirements of the state of TN & following Federal Food Codes. Supervisory duties include being responsible for the management of dietary staff & the function of the department in the absence of the Dietary Director.

Dietary Aide - back to top
Dietary Aide is responsible in aiding the Cook in insuring meals are prepared and served in a timely manner and in accordance with facility policies, physician orders, resident care plans, appropriate regulations and the recommendations of a Registered Dietitian. Duties include food prep, tray and tray line set up, dining room and dish room set up and break down, cleaning and sanitation of all areas in the dietary department. Other duties as assigned. Maintenance Technician - back to top
Maintenance Technician is responsible for assisting Maintenance Director in maintenance duties to include but not limited to interior maintenance as well as exterior maintenance. Duties include remodeling projects, i.e. drywall, painting, wallpapering, framing, plumbing, floor coverings, electrical, carpentry, landscaping and maintenance of work tools. Responsible for documenting work performed & audits & maintaining facility audit books as dictated by the Maintenance Director & Administrator to include but not limited to fire drills, fire extinguisher audits, monthly, quarterly, semi-annual & annual equipment cleaning & maintenance as applicable to the facility, state and/or federal standards for nursing homes.

Restorative Nurse Aide - back to top
The Restorative Nurse Aide is responsible to assist with restorative programs as indicated through range of motion exercises, positioning, ambulation, activities of daily living, dining & other programs designed to enrich the lives of the residents of the facility. Must be a certified nurse aide in the state of TN or hold certification as a nurse aide in a transferrable state per the State of TN guidelines. The Nurse Assistant must have a genuine interest in the care of nursing home residents, a cheerful attitude, patience, understanding & use tact & courtesy in addressing residents, families, staff & visitors. Lead Restorative C.NA - back to top
Lead Restorative Certified Nurse Aide (C.NA) assists with restorative programs as indicated through range of motion exercises, positioning, ambulation, activities of daily living, dining programs, etc. Will be responsible for keeping documentation of resident weights. Will attend NAR (Nutrition At Risk) meetings, collaborating with Dietary & Nursing in order to establish care plan for optimum outcome of resident status. MUST have former experience creating spreadsheets/reports using Microsoft Excel & Word. MUST have current certification, i.e. C.NA & in good standing. Experience in Restorative preferred.

The Activity Assistant, under the general supervision of the Activity Director, is responsible to assist in planning, organizing & implementing all functions of the Activity Department to meet the standards of regulatory agencies & of this facility & the individual needs of its residents.

Speech Therapist - back to top
Speech Therapist is responsible to screen, evaluate, re-evaluate, discharge plan & make revisions in any established plan of care as supervised by the physician in a professional, competent manner, upholding excellence with quality of care. Must have a degree in Speech Language Pathology from an accredited program. Must have an active licensure & in good standing. Must have excellent organizational & time management skills. Must have knowledge of code of ethics & role delineation, including guidelines for supervision. Must have active CPR certification.

Social Services Assistant - back to top
Follows the direction of the Social Services Director to meet the objectives of the nursing facility as part of the resident total care. Assures the maintenance of appropriate records of department services & submits to Social Services Director/Administrator as required.

Environmental Services Aide (Laundry Aide) - back to top
ES Laundry Aide is responsible for laundering linens & garments. Regulates flow of work to insure adequate supplies of clean linens for the facility. Responsible for the overall cleanliness of assigned work areas. Tasks include but are not limited to receiving soiled & returning laundered linens & garments multiple times throughout shift, inventorying linens & resident personal garments & laundry chemicals. Cleans filters for dryers each cycle, mops floors, dusts equipment & fixtures, disposes of trash, disinfects & deodorizes areas as needed, stocks assigned linen carts & other duties as deemed necessary by the department Director or other managers. Required to follow the schedules, policies & procedures of the department & the facility. Must be able read & write & follow verbal & written instructions.

Physical Therapist - back to top
